Taking a Medical Gynecology History the Proper Way

Menstrual History

Last Normal Menstrual Period
Age of menarche (or menopause)
Cycle characteristics - duration of menses, regularity, flow (heavy or light), associated pain
Any bleeding between cycles
Any mood changes
Any bleeding after intercourse

Pap

Pap Smear - last one? results? ever had any abnormal results? if yes, what treatments and/or follow-up?

Obstetrical History

GTPAL (Gravidity, total number of all pregnancies, Term Pregnancies greater than 37 weeks, Premature infants delivered between 20 to 37 weeks, number of Abortions, induced and spontaneous before 20 weeks, and Living children count)

For all pregnancies, ask: When, where, what (male/female, gestational age, weight), how (mode such as Caesarian or vaginal, duration of labor)
Any complications?

Sexual History

Age of first sexual contact (including oral, anal and vaginal)
Number of partners
Sex of partners
Current relationship, partner's health
Pain with intercourse
Bleeding with intercourse, post-intercourse bleeding
Any past history of assault or abuse

Contraceptive History

What (including past and present)
Do you ever forget to take or use it?
Any failures? Side-effects?

Past Gynecological Infections

Any pain? Discharge? Itch?
Past Infections? Yeast? STDs? PID? Vaginitis? Vulvitis?
How were these treated?
